Form A is required for all activities with the church or diocese. Make sure when filling it out the on page 2 that you can only sign 1, 2 or 3 and only sign 4a or 4b. Also for this form to be valid we must have a photocopy of your insurance card attached to the form.

Form B

Form B is required for all activities and is event specific. A new form B must be filled out for every event that your child attends. Below you will find links to B forms for our upcoming events.
